How do I initiate a Claim?

As we offer multiple product, the digital claims form will be specific to your coverage.

Please visit our Customer Portal and login or create an account with the email address associated with your policy/certificate. Once logged in, you'll see your coverage details and the instructions to submit your claim.

If you can't remember which email address is associated with the coverage you purchased and you have multiple emails, you may need to try a few until you see the policies/certificates displayed in the portal.
