What do I need to File a Claim

When submitting a claim, you will be required to provide documentation to support a claim.

Depending on the situation of the claim and the policy/certificate you have, the required documentation will vary.

For Refund Protection for a sports program, you can expect to provide the following:

  • Record/Proof of the Incident - Documentation to support the reason for no longer being able to participate - for example: medical documentation for an injury or illness, or a letter from a coach or program administrator describing the incident when the injury occurred.
  • Proof of your Registration - receipt showing your registration amount
  • Proof of Participation - copy of a roster or documentation from the team/league showing participation in the program
  • Copy of the Program Schedule - copy of the program/team's schedule or calendar

For Accident Medical, you can expect to provide the following:

  • Proof of Participation - copy of a roster or documentation from the team/league showing participation in the program
  • Documentation of the Injury Incident - note from the coach or program administrator confirming the date of the incident and that it occurred to your participant while participating in a sanctioned program activity. 
  • Medical Receipts documenting your out-of-pocket expenses related to your participant and the injury that experienced.
