When submitting a claim, you will be required to provide documentation to support a claim.
For Accident & Injury, you can expect to provide the following:
- Proof of Participation - copy of a roster or documentation from the team/league showing participation in the program
- Documentation of the Injury Incident - note from the coach or program administrator confirming the date of the incident and that it occurred to your participant while participating in a sanctioned program activity.
- Medical Receipts documenting your out-of-pocket expenses related to your participant and the injury that they experienced.
